Process safety risk refers to the potential for accidents and incidents that can result in harm to people, the environment, or property due to failures in industrial processes.

Examples of Process Safety Risks

• Chemical Releases: Accidental release of hazardous chemicals can lead to fires, explosions, and toxic exposures. For example, the Bhopal gas tragedy in 1984 was a catastrophic chemical release incident.
• Equipment Failures: Mechanical failures, such as pump or valve malfunctions, can cause leaks or ruptures. The Texas City refinery explosion in 2005 was partly due to equipment failure.
• Human Errors: Mistakes made by operators, such as incorrect valve operations or failure to follow procedures, can lead to dangerous situations. The Piper Alpha disaster in 1988 was exacerbated by human errors.
